Hi Marlene,
Most people select "prove it" for all of the paragraphs.
When you're being sued for a debt, the debt collector needs to prove you owe the debt. You don't need to prove you don't owe it. So, requesting the debt collector prove everything is what most people and most attorneys do.
Frequently, debt collectors can't prove the person they're suing owes the debt, so they drop the case.

There are no questions usually asked in the Summons. The Complaint has numbered allegations. These are assertions, or statements, not questions. In the Answer, you say whether you agree (admit) or disagree (deny) with the statements.
Here's an example of Complaint in California. The numbered statements are highlighted. Those are what you need to respond to.
